Program Components

Transportation Needs Assessment

Each participant’s mobility and transportation needs are assessed to determine appointment frequency, medical transportation requirements, and assistance needed for essential errands.

Medical Appointment Transportation

Transportation is coordinated for primary care, specialty visits, dialysis treatments, and VA medical appointments through contracted providers.

Essential Errands Transportation

Transportation is arranged for grocery shopping, pharmacy visits, banking, and personal care services to support daily living and autonomy.

House-Call Medical Coordination For individuals unable to travel, Infinity Cares coordinates house-call services through physicians and nurse practitioners.

Transportation Provider Oversight

Infinity Cares manages scheduling, communication, and coordination with transportation providers to improve reliability and continuity of service.
