Program Summary

The Healthcare Coordination & Benefits Navigation Program supports individuals in accessing, managing, and maintaining healthcare and public benefits essential to long-term stability and well-being.

The program provides hands-on coordination with healthcare providers, insurance plans, and public benefit systems, including Medicare, Medicaid, and Veterans Affairs. Services are available independently or as part of a comprehensive care plan.

Program Components

Healthcare & Benefits Assessment

Assessment of healthcare coverage, provider access, benefits enrollment, and unmet medical needs.

Primary Care & Provider Coordination

Connection to primary care providers, coordination with nurse practitioners, facilitation of medical record transfers, and post-discharge monitoring.

Medicaid & Medicare Navigation

Assistance with enrollment, recertification, plan optimization, and education on available benefits.

Veterans Benefits Navigation

Support with VA enrollment, healthcare access, and referrals for Aid & Attendance and long-term planning.

Advocacy for Individuals Unable to Self-Advocate

Acting as a point of contact for individuals with sensory, cognitive, or medical limitations. Ongoing Monitoring & Coordination Continuous oversight of care transitions, coverage status, and service continuity.
